Child population health is the measurement and distribution of health outcomes for all children, including the influence of demographics, multiple determinants of health, improvement interventions, and healthcare policies on measured health outcomes. Improving population health is one of the components of the Quintuple Aim.
Through multi-sector partnerships, VCHIP’s population health projects use health-related, statewide, data sources to assess child health outcomes to understand healthcare needs, explore inequities, plan for quality improvement, and evaluate interventions and policies, with the goal of optimizing the health and well-being of Vermont’s children.
