Patient Scheduling Forms

Welcome to our Internet scheduling information system. You may schedule via these forms at any time. If you have questions, please contact the CRC, (802) 847-2793, Monday through Friday, 7:00 a.m. to 2:00 p.m.

Any visit involving controlled research meals must be scheduled at least one week in advance.  Other visits need to be scheduled at least one business day in advance.

No subject may participate in any phase of a study unless there is a current signed informed consent on file at the CRC.

The scheduler will reply back to you by e-mail either to confirm or to suggest an alternative date and time if we cannot accommodate that visit.

Note, the forms look a little different now, but request the same information.  Outpatient and Extended outpatient scheduling now use the same form. We have added a "Change to existing appointment" form.
