Overview
The Blueprint Annual Conference is a yearly opportunity for Blueprint team members from around the state – and our colleagues working in primary care, population health, mental health and substance use disorder treatment services, and other areas of the health and human services systems – to gather and think together about what’s next.
This year, the conference will consider new opportunities for advancing population health, building on Vermont’s strong foundation of primary care and community collaboration. We’ll explore:
- Primary care’s role in population health – national trends and local perspectives
- Innovations in complex care
- Community-led reinvestment strategies with long-term impact
- New approaches to sourcing and sharing data in service of population health
Objectives
- Explore the role of primary care in population health, including national trends and local perspectives
- Learn about innovations shaping the practice of complex care
- Understand how some communities are organizing to plan reinvestments of health systems savings in population health projects, with an eye to long-term outcomes
- Encourage new thinking about how we might collect, share, and use data in service of population health
About the Blueprint
The Vermont Blueprint for Health designs community-led strategies for improving health and well-being.
Current Blueprint programs include Patient-Centered Medical Homes, Community Health Teams, the Hub & Spoke system of opioid use disorder treatment, the Women’s Health Initiative, Support and Services at Home (SASH), Self-Management and Healthier Living Workshops, full population data and analytics for policy makers, communities, and practices, and a series of learning labs for providers and community teams.

ACCREDITATION
In support of improving patient care, the Robert Larner, M.D. College of Medicine at the University of Vermont is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
The University of Vermont designates this live activity for a maximum of 5 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
The program has been reviewed and is acceptable for up to 5 Nursing Contact Hours.
As a Jointly Accredited Organization, The Robert Larner, M.D. College of Medicine at the University of Vermont is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provinicial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The Robert Larner, M.D. College of Medicine at the University of Vermont maintains responsibility for this course. Social workers completing this course receive 5 continuing education credits.
This program is approved for Allied Mental Health Counselors for up to 5 continuing education credits.

Fees
$85 General Admission (for those not requiring continuing education credit)
$100 Nurses and Other Health Care Professionals (includes continuing education credit)
$130 Mid-level providers (NP's, PA's, etc.) (includes continuing education credit)
$160 Physicians (includes CME credit)
Fee includes conference materials, continental breakfast and lunch.
Refunds: Refunds, minus an administrative fee of $50, will be made only if notice of cancellation is received in writing by April 19, 2019. No refunds are possible after that date.
2019 Fee Policy for the Vermont Blueprint for Health Conference
There will be no exceptions to this policy.
Blueprint field staff (Project Managers, CHT Leads, Spoke Staff, Regional Coordinators etc.) will pay the full General Admission registration fee supported by the “Training, Travel, Flexible Fund” line of Health Services Area Grants.
State employees of Agency of Human Services (DVHA, DAIL, VDH, DOC etc.) pay registration fee in full.
UVM will no longer bill your State of Vermont department after the conference. State employees may register online by using a credit card or by providing a State of Vermont Purchase Order Number. Purchase Order numbers are required.
