Larner College of Medicine Alumni Association Award Nomination Form
The LCOM Alumni Association Awards give recognition to medical and PhD/MS alumni who have been outstanding in their service to the College, the community at large, or their career accomplishments. Strong consideration is given to nominees who demonstrate outstanding commitment to equity in medicine, reflecting the College's mission "to benefit Vermont and the world through engagement with diverse communities."
We invite you to take this opportunity to nominate an alum of the UVM College of Medicine- perhaps a fellow colleague, classmate or former student -- for one of these prestigious awards.
Nominations are now being accepted for the 2023 Awards Cycle. The Medical Alumni Association Award recipients will be recognized during Larner College of Medicine Reunion 2023.
In order to submit a nomination - the following steps must be completed:
1. Online form linked below must be submitted.
2. Two ( 2 ) Letters of support for the nominee must be included which specifically outlining the reason for nominating. Send letters to Cristin Gildea at Cristin.Gildea@uvm.edu or by mail at Given Building, N-329, 89 Beaumont Avenue, Burlington, VT 05405.
3. The submission of your nominee’s curriculum vitae and other supporting information. **Nominators will be asked to help gather additional biographical information and/or assist in writing award citations for their selected nominees. Nominees not chosen for an award may be considered in future years.
